WASHINGTON An estimated 2.2 million people in the United States live with epilepsy, a complex brain disorder characterized by sudden and often unpredictable seizures. The highest rate of onset occurs in children and older adults, and it affects people of all ethnicities and socio-economic backgrounds, yet this common disorder is widely misunderstood. Epilepsy refers to a spectrum of disorders with seizures that vary in type, cause, severity, and frequency. Many people do not know the causes of epilepsy or what measures to take if they witness a seizure. A new report from the Institute of Medicine highlights numerous gaps in the knowledge and management of epilepsy and recommends actions for improving the lives of those with epilepsy and their families and promoting better understanding of the disorder.
Effective treatments for epilepsy are available but access to treatment and timely referrals to specialized care are often lacking, the report's expert committee found. Reaching rural and underserved populations, as well as providing state-of-the art care for people with persistent seizures, is particularly crucial. The report's recommendations for expanding access to patient-centered health care include early identification and treatment of epilepsy and associated health conditions, implementing measures that assess quality of care, and establishing accreditation criteria and processes for specialized epilepsy centers. In addition, the wide variety of health professionals who care for those with epilepsy need improved knowledge and skills to provide the highest quality health care.
Some causes of epilepsy, such as traumatic brain injury, infection, and stroke, are preventable. Prevention efforts should continue for these established risk factors, as well as for recurring seizures in people with epilepsy and depression, and for epilepsy-related causes of death, the report says.
People with epilepsy need additional education and skills to optimally manage their disorder. Consistent delivery of accurate, clearly communicated health information from sources that include health care professionals and epilepsy organizations can better prepare those with epilepsy and their families to cope with the disorder and its consequences, the report says. Accurate, current data on the extent and consequences of epilepsy and its associated health conditions are especially needed to inform policymakers and identify opportunities for reducing the burden of epilepsy.
Living with epilepsy can affect employment, driving ability, and many other aspects of quality of life. The report stresses the importance of improved access to a range of community services, including vocational, educational, transportation, transitional care, and independent living assistance as well as support groups. The committee urged collaboration among federal agencies, state health departments, and relevant epilepsy organizations to improve and integrate these services and programs, particularly at state and local levels.
Misperceptions about epilepsy persist and a focus on raising public awareness and knowledge is needed, the report adds. Educating community members such as teachers, employers, and others on how to manage seizures could help improve public understanding of epilepsy. The report suggests several strategies for stakeholders to improve public knowledge of the disorder, including forming partnerships with the media, establishing advisory councils, and engaging people with epilepsy and their families to serve as advocates and educators within their communities.

Prostate cancer vaccine Provenge has long incited passions unlike any other cancer therapy.
Doctors who raised doubts about it received death threats. Health regulators and lawmakers faced loud protests at their offices. A physician at the American Cancer Society was so intimidated by Provenge partisans that he yanked a skeptical discussion of it from his blog.
The vitriol dissipated in April 2010, when the U.S. Food and Drug Administration approved Provenge for advanced prostate cancer, satisfying investors in manufacturer Dendreon and patients who for years had demanded it be put on the market.
But the bell on Round Two sounded when Marie Huber, a trained scientist and former hedge-fund analyst, made it her mission in the last year to analyze what she believes are deadly flaws in the studies that led to the approval of Provenge by the FDA.
She argues that the main reason Provenge seemed to extend survival - a crucial factor in the FDA?s decision - was that older men in the study who did not receive Provenge died months sooner than similar patients in other studies.
She raises the possibility the ?placebo? they received was actually harmful and made Provenge, known scientifically as sipuleucel-T, look better by comparison.
As Huber gains traction, most notably with a February paper in the prestigious Journal of the National Cancer Institute, she, too, is receiving threats. One post on an investors? message board last month suggested that ?somebody smack her with a rubber hose.? An email said ?don?t think you will be unscathed in this battle you waged on Provenge.?
Provenge is Dendreon?s only product and the company?s stature with investors has waned with disappointing sales. In 2011, product revenues totaled $213.5 million, far from the $400 million Dendreon initially projected.
The company insists Huber?s analysis is flawed and that Provenge has helped thousands of men with prostate cancer.
?I?m looking forward to getting this to patients around the world,? said President and Chief Executive John Johnson.
FIRST CANCER VACCINE
Since it won FDA approval two years ago, Provenge has been Exhibit A for the idea that a patient?s immune system can control or cure cancer. The first therapeutic cancer vaccine to reach the market, Provenge tries to engineer white blood cells, part of the immune system, to vanquish prostate cancer, which killed an estimated 33,720 men in the United States last year.
Its path to approval has all the features of a heavyweight healthcare fight - desperate patients demanding access to a promising therapy, a very expensive drug that extends life only a few months and efficacy data open to interpretation.
The FDA declined to approve the drug in 2007, when a clinical trial failed to show it slowed tumor growth. That incited protests, lawsuits and death threats against physicians on the FDA advisory panel who did not recommend approval, breaking with the 13-4 majority in favor.
?Provenge came along when we didn?t have much to offer for prostate cancer,? said Dr. Len Lichtenfeld of the ACS. ?The advocacy community was bursting at the seams for something that worked. When you have that situation, it inflames passions and that can overtake the science.?
In the pivotal trial called IMPACT, published in July 2010, but shared with the FDA months earlier, Provenge extended median survival by 4.1 months to 25.8 months from 21.7 months. That was sufficient for FDA approval. The vaccine costs $93,000 and patients also incur physician and other charges. Medicare agreed to cover Provenge last year, as have private insurers, but doctors initially balked at a long wait for reimbursement.
Huber had long been ?utterly intrigued? by Provenge and its ?huge promise of harnessing the immune system to battle cancer,? she said in an interview.
In documents JNCI requires authors to sign, she declared no financial conflicts of interest. Neither she nor her former firm nor anyone else she is connected to stands to benefit financially from her analysis, she said.
Instead, she says she is motivated to help ?vulnerable and desperate patients? - so much so that she gave up her job, salary and health insurance. As I watched the final few minutes of Wrath of the Titans, I thought, "What I wouldn't give to be a 10-year-old boy right now."
That's absolutely the audience that will enjoy this silly, overblown special-effects extravaganza. At 10, you can still buy into the reality of wild fantasy; at that age, you live for the shocks and sensations of this kind of movie. It's the 10-year-old's meat-and-potatoes: movies built around explosions, outlandish monsters, fireballs and creatures made out of magma that drip lava. Oh yeah - and the clanging of swords.
Unless you're that age - and that undiscriminating - well, Wrath of the Titans is an unholy bore. Its most regular source of amusement is the clunky dialogue of Dan Mazeau and David Leslie Johnson and the sight of a panoply of otherwise-serious actors sporting the flowing robes and tresses of Hebrew prophets or, in this case, ancient Greek gods.
Ostensibly a sequel to 2010's similarly silly Clash of the Titans, with several of the same actors reprising roles, Wrath echoes last year's Immortals, offering the further adventures of demigod Perseus (Sam Worthington), who must stop the escape of the evil Titans - imprisoned in the Underworld - who want to overthrow the gods and destroy the Earth.
(Immortals actually dealt with Theseus - but the makers of this film give Perseus a couple of his adventures, including a battle against a minotaur.)
In this mythical mash-up, Ares (Edgar Ramirez), petulant god of war and unhappy son of Zeus, teams with the evil Hades (Ralph Fiennes) to drain the powers from Hades' brothers Zeus (Liam Neeson) and Poseidon (Danny Huston). In doing so, they'll set free Cronos, the evil titan who was father to Zeus, Hades and Poseidon. Got that?
When unleashed, Cronos will obliterate humanity, though Hades reconciles his role as Cronos' accomplice by noting that Cronos has promised him immortality. It feels as though these guys haven't really thought this through: If they destroy Earth, well, do the math.
It's up to Perseus, half-human son of Zeus, to team up with Queen Andromeda (Rosamund Pike) and fellow demigod Agedor (Toby Kebbell) to stop Ares and rescue Zeus. Do you doubt they will?
But director Jonathan Liebesman mistakes motion for action. His camera follows his heroes doggedly as they dodge giant rocks, rampaging cyclopses and flying fireballs, but the jumpy editing drains the imagery of tension, rather than enhancing it.
And here's the other thing about bad CGI-powered movies: The filmmaker is generally afraid to actually let you focus on any of his digital creations too long, lest the seams show. It doesn't matter whether it's a giant cyclops, a pro-wrestler-style minotaur or Cronos' double-torso warriors: He never stops to let you get a fix on what they actually look like. Consider the difference between trash like this and something like Avatar, where James Cameron gave the viewer long, loving looks at his all-CG images.
The 3D? Unnecessary as usual (unless you have allergies and are easily upset by images of floating dust motes that pop of the screen).
But then Wrath of the Titans is itself totally unnecessary. Unless you're a 10-year-old boy.

Pervasive Computing Technology: A Glimpse of the Next Generation Computer

What is pervasive computing? It all started in 1991 when Mark Weiser envisioned the next generation computer that weaves themselves into their environment. The next generation computers make themselves invisible and intuitive to use. Computers disappear into the background. If you?re a Star Trek fan, think of the computers in this science fiction series, and you?ll get the idea.
The term coined by Weiser, ubiquitous computing, is now synonymous to pervasive computing.
Computers as they exist today do not integrate themselves into our environment. You?re aware of the fact that you?re using a computer. The PC sits on top of your desk. You carry a laptop or pocket PC around. You need to acquire certain skills in order to use them, e.g. if you want to use a spreadsheet, you have to learn to use it.
Weiser envisioned an environment where computers are integrated into the environment in such a way that users are not even aware of the computers, or that they?re using one. The computers disappear and become unobtrusive.
To illustrate the point, Weiser and his colleagues invented 3 types of devices: a tab, a pad and a board. The devices have no individualized association in they way that you associate a PC or laptop to a particular user, e.g. you can grab a tab and use it without having to figure out how you?re going to configure or personalize it to your needs. A tab is a handheld device and can be easily carried around. You can use it to access your email, chat, or to pop up a reminder for you to ask a colleague to confirm next week?s seminar if you run into him in the hallway. You?d have dozens of this in your environment.
A pad is kind of a digital version of a paper. You can spread it on your desk in the same way you spread papers on your desk.
The digital board replaces today?s whiteboard. When a group of people gather in a room, the system detects that they belong to a project group and automatically downloads the previous discussion points on the board. You can write on the board using a digital pen while sitting a few meters away from it ? just point and write.
Pervasive computing integrates computers and a smart environment, and blends them into the background. This technology is changing the way we work, live, and interact with each other.
Let?s look at a few more examples of applications of this technology.
Cowboys on horsebacks herding cattle might one day become a feature of a bygone era as the introduction of virtual fences allows farmers to herd their cattle from the comfort of their homes. The virtual fence is downloaded to the cows by transmitting GPS coordinates to head-collars worn by the cows. The dynamic virtual fences are moved along desired trajectories. The collars are equipped with a Wi-Fi networking card, a Zaurus PDA, an eTrex GPS unit and a loudspeaker that transmits occurring sounds (for example, roaring tigers, barking dogs) when a cow strays from the intended path. This multi-disciplinary project, the brain child of a biologist, is made possible in collaboration with computer scientists.
Sensor technology can potentially play an important role in search and rescue operations by first responders, i.e. emergency personnel, such as firemen, paramedic, and police, who arrive at the scene immediately after an event (e.g. a fire, an earthquake, a building collapse) occurs.
Firemen wear tags to allow easy tracking of their movement in order to coordinate search and rescue operations more effectively. The firemen can be informed if a particular section of a building is found to be unstable and is about to collapse, and is directed to evacuate it immediately. A wireless vital sign monitor is attached to victims found trapped so that their condition can be monitored in order to ensure that they receive the appropriate medical attention as soon as they are rescued.
This non-invasive sensor monitors vital signs such as heart rate, oxygen saturation and serum chemistry measurements. The vital sign monitor helps the paramedic team determine which victims are in more critical conditions so that they can prioritize medical attention to more severely injured victims. The application and architecture required to support this emergency response application is being developed under the CodeBlue project at Harvard University, USA.
Wireless technology is also used in healthcare. The Arrhythmia Monitoring System (AMS) is a medical telemetry (telemedicine) system that makes use of wireless technology to monitor patients suffering from arrhythmia. Among the complications that arise from arrhythmia are the loss of regular heartbeat and subsequent loss of function, and rapid heartbeats.
AMS provides a means for healthcare professionals to continuously monitor a patient?s electrical cardiac rhythms remotely even though the patient is not at the hospital. This technology allows patients to be in the comfort of their homes without jeopardizing their health. It is also useful for monitoring the heart functions of astronauts who are more susceptible to cardiac dysrhythmias when in space.
